Job Summary
The community facilitator is responsible for directing community-related activities and coordinating the dissemination of services and activities provided by both the mobile medical team and community health workers (CHWs) in the target community. This role is essential for the proper development of the activities of the iCMM strategy that this project will implement.
Key Responsibilities
- Together with the Program Coordinator and the CHW, identifying the needs in the target population and participating in the definition, planning and updating of the appropriate iCCM strategy and the mobile health activities.
- Performing initial data collection and establishing base line criteria, analyzing data and mapping the potential problems.
- Participate in the creation of any materials necessary for the activities of social workers and community health workers, so that the material can be adjusted to the context, cultural sensitivity and needs of the community.
- Liaise and develop relationships with medical, welfare, educational, vocational, mental health, and other potential community services regarding referral and service provision for the targeted communities
- Assist in establishing and maintaining efficient referral processes to improve access to advanced care.
- Support the referral of patients to private hospitals for advanced tests or treatments, working closely with stakeholders to ensure a holistic approach to patient care.
- Mapping and regular update of partners, local authorities, other NGOs, and governmental social services in order to improve the services scope and responsiveness.
- Keep records, statistic and files updated, reporting any social activity, in order to prioritize and develop appropriate projects.

Education / Experience Needed
- Degree in Public Health, Nursing, Social Work, or a related field preferred.
- Two years minimum in a similar social/community job
- Experience in training and supporting community health workers or similar roles is highly desirable.
